Orthopaedic Specialty Institute Medical Group of Orange County outsources our release of information process to HealthMark Group. To follow up on the status of your medical record request, you may contact HealthMark Group directly at 800-659-4035 or [javascript protected email address].

You may submit a request electronically with HealthMark Group by visiting healthmark-group.com/release-of-information/for-patients/

Here’s how it works.

1. Enter your email
2. You’ll receive a secure link to that email to access Request Manager (no username and password to remember – whew!)
3. Follow the step-by-step instructions on the screen to submit your request
4. We will process your request, and you will receive an email with a link to download your records once the request is complete
